Florida Department of Health will be providing vaccines at the Sarasota Square Mall (new location)

The Florida Department of Health in Sarasota is moving their vaccine clinic location to Sarasota Square Mall. Centrally located to serve all of Sarasota County residents.

You can access the vaccine location through the old Sears entrance and the main entrance used by buses.

CONSENT FORMS ARE NEEDED FOR SECOND DOSES because it’s a vaccination appointment. The consent form addresses things that may have changed since your first appointment like how you’re feeling, medical conditions, and so on.
